Incident, on A49 at Wooferton around 13:20 saw the road between A456 and A4117 near Ludlow closed and detour put in place.
The West Midlands Ambulance Service said it was called to the scene at 1:13 p.m.
A spokesman said two ambulances had arrived, a paramedic and a Midlands ambulance from Strensham.
Drivers of both vehicles were injured – driver of van seriously.
A spokesman for the West Midlands Ambulance Service stated: driver of van, man, was out of in vehicle when the carriages arrived and was found get seriously injured.
“He was treated on scene before being transported by ground ambulance on the blue lights of the Royal Shrewsbury Hospital for further treatment of injuries.
“Truck driver, male, treated on scene for less serious injuries and was taken to Hereford Hospital. for further checks.
West Mercian Police said officers were called to crash near Ashford Bowdler around 13:20 and blocked the road.
The representative said one of in drivers suffered “a serious but life-changing injury”.
Motorways England said the diversions were staged in place in in both directions until the road was reopened around 17:30.
